Home Assessments

We offer our residents and their families the opportunity to have a Home Assessment prior to the resident returning home. The Home Assessment is performed by a Physical and Occupational Therapist to evaluate the resident's abilities to function in their own environments, to assess the set-up of the home environment, and to make recommendations to increase safety and independence. This also allows each therapist to modify the plan of care and goals specific to the individual.

Out-Patient Therapy

Once residents return home, they are able to continue their out-patient therapy at our facility to assist with a smooth transition and continued progress.  Out-patient Physical, Occupational and Speech Therapy are all available 3-5 days per week. Out-patient therapy is also available to individuals who have not received in-patient therapy at our facility.  Please contact our facility Business Office to determine if we accept your insurance.  Upon request, transportation is available in the Cleveland area free of charge.
